Singer begins with the Greeks, whom he identifies as the founders of scientific medicine. The Hippocratic school, centered at Cos and Cnidus, established the empirical method: careful observation of patients, cautious inference, and the doctrine of the healing power of nature. The Hippocratic Collection — a corpus of roughly a hundred works attributed to multiple authors across several centuries — embodies this spirit, offering case records of extraordinary precision and an ethical ideal crystallized in the Hippocratic Oath. Aristotle extended biological inquiry through comparative anatomy and embryology, codified the doctrine of the four humors and four elements, and placed the heart at the center of life — an error with lasting consequences. At Alexandria, Herophilus and Erasistratus advanced anatomy and physiology, respectively: Herophilus identified the brain as the seat of intelligence; Erasistratus developed a pneumatic physiology of veins, arteries, and nerves. Galen of Pergamum synthesized these traditions into a vast theoretical system grounded in teleological anatomy and a three-spirit physiology (natural, vital, and animal spirits) that would dominate medicine for fifteen centuries. With the collapse of Rome, rational medicine entered a long decline. The Salernitan school in southern Italy preserved a faint continuity, while Arabic scholars — Rhazes, Avicenna, Albucasis, Isaac of Kairouan — translated, extended, and in some cases improved upon Galenic medicine. Their works, rendered into Latin by translators working in Spain and Sicily, reawakened European intellectual life and furnished the medieval universities with their medical curricula. At Bologna, Mondino di Luzzi performed actual human dissections and wrote the first modern anatomical manual. Guy de Chauliac produced the dominant surgical text of the later Middle Ages. Institutional developments of the period included the hospital system — derived from Roman military valetudinaria and elaborated by Christian charity — and the innovation of quarantine, originating at Ragusa, which Singer credits as one of the few genuine medieval advances in preventive medicine. The Renaissance transformed anatomy decisively. Leonardo da Vinci interrogated Galenic physiology through direct observation, questioning the cardiac septum and pulmonary theory, though he never published. Andreas Vesalius of Brussels, appointed professor at Padua at twenty-four, produced *De Humani Corporis Fabrica* in 1543 — the first great modern scientific monograph, founded entirely on direct dissection. Singer ranks this work alongside Copernicus's astronomical treatise as a joint demolition of medieval cosmology. Ambroise Paré simultaneously reformed surgery: abandoning boiling oil for gunshot wounds, introducing ligature in place of cautery, and elevating the surgeon's professional standing. William Harvey's demonstration of the circulation of the blood (published 1628, worked out by 1615) constitutes, for Singer, the pivotal event of post-Renaissance physiology. By calculating the volume of blood ejected per hour and showing that the valves in veins and arteries enforce unidirectional flow, Harvey proved that blood circulates continuously. Malpighi completed Harvey's work by discovering capillary vessels under the microscope. Leeuwenhoek, Swammerdam, and others opened microscopic analysis of the animal body. Meanwhile, Sanctorius introduced measurement into physiology; Boyle established chemistry as an independent experimental science; Mayow identified the respiratory fraction of air; and Lavoisier ultimately demonstrated that respiration is a process of oxidation in the tissues. The eighteenth century consolidated these gains through clinical teaching. Boerhaave at Leyden — Singer's nominee for greatest physician of modern times — united bedside instruction, post-mortem correlation, chemistry, botany, and anatomy into a coherent medical education that founded the Edinburgh school and much of Anglophone medical training. His pupil Haller produced the first modern physiology textbook, distinguishing muscular irritability from nervous force and placing sensation firmly in the nerves. Morgagni at Padua founded morbid anatomy by correlating symptoms with post-mortem lesions across tens of thousands of cases. Laënnec introduced the stethoscope and with it systematic physical examination of the chest. Jenner's vaccination against smallpox opened the entire field of immunology. Preventive medicine underwent its own revolution. Pringle, Lind, and Cook applied sanitary principles to armies and navies. Chadwick and Southwood Smith, inspired by Bentham's utilitarian philosophy, drove British public health legislation — the 1848 Public Health Act, vital statistics registration, medical officers of health — producing administrative frameworks that other nations subsequently adapted. Parallel developments in the United States evolved from the Marine Hospital Service into a federal public health apparatus. The final phase Singer describes is scientific specialization: the subdivision of medicine into disciplines each pursuing general laws through experimental method. Pasteur and Koch established the germ theory of infectious disease; Lister introduced antiseptic surgery; physiological chemistry, pathology, and epidemiology became independent sciences. Singer frames this not as fragmentation but as the maturation of the Hippocratic method — systematic observation, cautious inference, and the subordination of theory to evidence — operating now at a cellular and chemical level rather than at the bedside. Throughout, Singer's organizing argument is that rational medicine is neither a modern invention nor an ancient inheritance passively received, but a tradition repeatedly recovered, corrupted, and renewed — and that its essential spirit, embodied first by Hippocrates, consists less in any particular discovery than in the willingness to learn from nature rather than from authority.
